JD Health and Wellness Center Awarded Tier 5 Recognition by the Oregon Health Authority

The Oregon Health Authority (OHA) has officially awarded Tier 5 Patient-Centered Primary Care Home (PCPCH) recognition to JD Health and Wellness Center, marking the clinic as one of Oregon’s top-performing community health organizations in patient-centered care.

This distinguished recognition, effective August 12, 2025, reflects JD Health and Wellness Center’s excellence in providing high-quality, person- and family-centered care that prioritizes whole-person wellness, accessibility, and community trust. Tier 5 is the highest level of recognition granted under Oregon’s PCPCH model — a standard that measures how clinics deliver coordinated, comprehensive, and culturally responsive care to their patients.

“Achieving Tier 5 PCPCH recognition is a reflection of our deep commitment to community-driven, patient-centered health,” said Eric Davis, Director of JD Health and Wellness Center. “Our team works every day to meet people where they are — combining preventive care, wellness services, and compassionate support that empowers individuals and families to thrive.”

As part of this recognition, JD Health and Wellness Center will be listed publicly among Oregon’s recognized Patient-Centered Primary Care Homes — highlighting the organization’s leadership in advancing equitable and integrated healthcare. The recognition remains valid through August 12, 2027, after which JD Health will be eligible for renewal or higher-tier evaluation.
